Initial screening Ax (1 1⁄2 hours) @150/hr
HCP Additional assessments (one-hour) = $150

NDIS

Initial screening Ax (1 1⁄2 hours) @ $190/hr

NDIS Additional appointments $190/hr
Extra time will be added per session will be charged pro rata.

Reports

Reports to Professionals = $190/hr NDIS Reports = $190/hr

HCP Reports=$150/hr

Feedback

Feedback session = $150/hr

Home Exercise Programme = $150/hr

Other Services

Additional services provided are charged separately : Additional reports, indirect non face to face clinical time, home visits, other professional liaisons and telephone calls lasting longer than 15 minutes will be charged for as a pro rata consultation. Travel charges depend on time and distance.

Cancellation policy

Each appointment is regarded as a firm commitment and missed appointments will be charged for. If you are unable to attend a session, a 48 notice period allows me to contact those on the waiting list to fill your appointment time as the practice does not operate on a “walk in/fill in” basis like a general medical practice.

NDIS Late Cancellation Fees (less than two business days) apply as per NDIS guidelines.

Private Client HCP Late Cancellation Fees (less than 24 hour notice) apply.
